Information on digitisation

The purpose of this notice is to explain matters relating to the digitisation of our patients’ paper medical records.
Pursuant to Article 13b of the Act of 28 April 2011 on the healthcare information system, a healthcare provider may convert medical records maintained and stored in paper form into electronic form, hereinafter referred to as ‘the digitisation of medical records’.
Voxel S.A., as a healthcare provider, digitises paper medical records generated in connection with the examinations it carries out. The digitisation process involves scanning the paper medical records, applying the appropriate electronic signature to them, and storing the records in electronic form.
Documents produced as a result of digitisation are equivalent to the originals.
The patient has the right to collect the originals of their paper medical records once they have been digitised. Refusal to collect the paper medical records will entitle Voxel S.A. to permanently destroy the paper records in such a way as to prevent the patient from being identified.
It should be borne in mind that, despite the digitisation of paper medical records, patients still have the right to access their medical records.
